First 5 Fox Valley was awarded a $100,000 PDG Community Innovation grant!
A highly competitive one-year grant implementing the nationally recognized evidence-based Help Me Grow model in Outagamie, Calumet, and Winnebago counties to promote cross-sector collaboration was awarded to First 5 Fox Valley. The model will help build on existing resources and strengthen relationships between partners. Additionally, it will expand development screening and assessments for infants and toddlers to ensure they receive the necessary supports needed to thrive. Grant activities include a:
- Centralized Access Point call-in center using the Help Me Grow database.
- Family Outreach Navigator to lead the ASQ developmental screening project providing free ASQ screenings to families.
- Help Me Grow Advisory Council to assure best practices and early intervention services across health care, early care and education, schools, and social service agencies are linking appropriate resources to support families with a “warm handoff” and on-going follow-up.
