Coordinated Funding for Collective Impact

By employing a community partnership model, First 5 Fox Valley has successfully harnessed the collective efforts of multiple organizations, working together in a harmonious and coordinated fashion. The fostering of community partnerships for collaborative and sustainable impact is a significant component of First 5 Fox Valley’s vision.  The grant funds secured by First 5 Fox Valley are distributed to empower various community partners for diverse projects.

At Family Services of Northeast Wisconsin, Help Me Grow dedicates resources to support a part-time navigator in Shawano County. Additionally, we collaborate on the Welcome Baby program, which involves hospital visitations and the assistance of a Help Me Grow navigator.

VPI- Early Intervention B-3 extends part-time Help Me Grow navigator support across three counties, including Calumet. This assistance is focused on families transitioning out of services.

LEAVEN & Casa Hispana partner with us to provide part-time Help Me Grow navigators, with a special focus on supporting bilingual families.

Catalpa Behavioral Health has integrated a part-time Help Me Grow navigator into its intake services unit.

At People of Progression, a part-time Help Me Grow navigator actively collaborates with the community advocate, contributing to the establishment of prenatal support for expectant parents to access new Doula services.

Children’s Wisconsin extends the reach of Help Me Grow with a part-time rural navigator serving Green Lake and Marquette Counties. Additionally, they have entered into a second contract to lead the expansion of the Triple P Strengthening Families training into the Fox Valley, serving as an anchor in the new Family Resource Center.Toggle Content

CAP Services dedicates part-time Help Me Grow navigator support to Waushara County.

In Waupaca County, Health Services incorporates a part-time rural Help Me Grow navigator. This navigator plays a vital role within the Healthy Beginnings program, serving various developmental screening needs within the community, child care, health, and school districts.

The ThedaCare Social Determinants of Health Team assigns a part-time Help Me Grow Navigator to lead the efforts in creating pathways for healthcare systems to access Help Me Grow community navigators. This includes work within EPIC and the Help Me Grow database.

Building for Kids provides essential support by offering enrichment activities for families entering the Welcome Baby program. This includes monthly parent engagement activities and complimentary 18-month memberships.

The Appleton Fox Cities Kiwanis actively supports the NICU Welcome Baby project by providing fiscal support and expertise in creating new parent bags for families facing birthing crises in NICU wards.

Help Me Grow has partnered with various child care programs, employing part-time navigators with expertise in child development and early learning. These navigators work with 53 child care programs, utilizing the ASQ Enterprise Developmental Screening System and offering ongoing family follow-up. They support a wide range of child care programs, including family in-home providers, center-based programs, Head Start, and School Districts across all 8 counties. We have four private contractors dedicated to working part-time with child care programs, focusing on improving the quality of these programs and advancing the YoungStar System.
